What is an Immigration Physical?
An Immigration Physical—also known as a Green Card Medical Exam, Permanent Residency Medical Exam, or Green Card Immigration Exam—is required by USCIS for all applicants adjusting their status to become lawful permanent residents. During this process, applicants must complete Form I-693 (also known as the immigration physical form or USCIS medical form) with the assistance of a certified civil surgeon. This form confirms that the applicant meets the health standards outlined by USCIS.

What to Expect During the Immigration Medical Exam?
Your visit includes all services required by USCIS:
- A complete physical examination
- Review of your vaccination records
- Administration of required immunizations (if needed)
- Tuberculosis (TB) screening
- Testing for gonorrhea and syphilis
What do you need in your Appointment?
To help avoid delays, please bring the following:
- A valid government-issued photo ID (passport or driver’s license)
- Completed Part 1 of Form I-693 using black ink only (do not sign Part 2)
- Proof of prior vaccinations or immunizations
- Your medical history and a list of current medications
- Payment – due at the time of your visit (we accept cash, debit, and credit cards)
- Interpreter, if you need assistance with English
**We recommend scheduling your exam just before submitting your I-485 application.Once signed by the civil surgeon, the I-693 form is valid for 2 years.
Vaccination Requirements for Immigration Medical Exams
Meeting the USCIS vaccination requirements is a key part of your immigration medical exam. These vaccination requirements vary based on your age and health status and are necessary to complete the I-693 form. If you do not have proof of immunization, you may need to begin new vaccinations. We can conveniently provide the necessary vaccines in-house at a competitive price. Alternatively, you can obtain the vaccines from other providers or pharmacies and submit proof of immunization afterward.
Required Vaccines for Adults:
- MMR (Measles, Mumps, Rubella)
- Varicella (Chickenpox)
- Hepatitis B
- Polio
- Tdap (Tetanus, Diphtheria, Pertussis)
- Influenza (Flu) – Required October through March
- Pneumococcal (Required for applicants 65+)
Children must show proof of all CDC-recommended childhood immunizations as part of the immigration medical exam requirements.
**COVID-19 vaccine is no longer required as of January 2025**

Required Lab Testing for Completion of Form I-693
Exam Cost and Coverage
The cost of the green card medical exam is $420 per adult or child applicant. This does not include vaccinations or vaccine titers to prove immunity. Payment must be made in cash, debit or credit card only.
** We do not accept medical insurance for all services required for the I-693 Immigration form.
What is included in the cost of Exam
A full physical by a USCIS civil surgeon.
Quantiferon (Tuberculosis) Test
Gonorrhea Test
Syphilis Test
Vaccination status verification
Copies of your laboratory results
Completion and sealing of Form I-693 (with a copy for your records)
- A TB blood test (QuantiFERON) for applicants age 2 and older.
Should the results come back positive, you will be referred for a chest X-ray at an external imaging facility.
Pregnant Applicants: If a chest X-ray is necessary, you must sign a consent form. We’ll guide you through the process to ensure you understand your options. - Gonorrhea Testing – Urine test required for applicants ages 18–24.
- Syphilis Testing – (RPR) Blood test for applicants ages 18–44.
What is NOT included in the cost of Exam
Please be aware that the following additional tests are not included in the total cost of $420 and are available for an additional fee:
- Vaccine titers
- Vaccinations
- Chest X-rays
- Seasonal Flu Shot (Required during the flu season (Oct 1 – March 31)
