DOT Physical Exam
The federal guidelines require all commercial drivers to pass the DOT health exam performed by a Certified Medical Examiner. This is done to ensure the safety of everyone on the road and to make sure potential drivers are physically capable of safely operating commercial vehicles.

Things You’ll Need
- A Valid Identification.
- A Medical Examination Report Form.
- A complete list of all your medications with the doses and your doctor’s name.
Be sure to complete Driver Information and Health History sections on the Medical Examination Report Form to save time.
As per the regulations of the Department of Transportation (DOT), drivers of large vehicles such as trucks and buses are required to undergo a physical examination to demonstrate their fitness and capability of controlling these machines on US public roads. This medical examination, also known as the DOT physical or the DOT Medical Exam and Commercial Motor Vehicle Certification, must be taken every two years.
DOT Physical Requirements
- 20/40 visual acuity (with or without corrective aids)
- 70 degrees or more of peripheral vision in each eye
- Hearing a ‘forced’ whisper 5 feet away without a hearing aid (or)
- Passing a formal audiometer hearing test
- Blood pressure at or below 140/90

Avoid These Before Your Exam
avoid consuming coffee, tobacco, and energy drinks for at least one day before your medical assessment. These substances can cause a spike in your blood pressure by up to 20 points. As per the guidelines of the Federal Motor Carrier Safety Administration (FMCSA), high blood pressure may have an impact on your certification period, and it may also prevent you from obtaining a new medical card or renewing an existing one.
What Comes After the Exam
A copy of the completed DOT medical examination form and a laminated copy of your medical card will be given to you on the same day of the examination. A copy of your drug test result, which typically takes 3 to 4 days to process, will be sent to the employer if applicable.
